This form may be completed by Orange Veins representatives, appointed officials, or relevant staff of host organisations to document safeguarding or discrimination incidents involving individuals or teams who have signed an agreement.
The form applies to:
– incidents that occur during tournaments, trips, or events where Orange Veins is responsible for the participation, supervision, or coordination of teams or individuals;
– situations where Orange Veins sends a team to an event organised by a third-party organisation.
The purpose of this form is to ensure a consistent and transparent approach to:
– recording incidents and immediate actions taken;
– assessing risks and determining appropriate follow-up measures;
– deciding whether escalation to host organisations, governing bodies, or external authorities is required;
– ensuring clear communication with all relevant parties.
All information will be handled confidentially in accordance with the Orange Veins Safeguarding and Anti-Discrimination Policy and Reporting Protocols. The form must be completed accurately, objectively, and in good faith.
